___FREQUENTLY ASKED QUESTIONS REGARDING EMPLOYEE NON-REVENUE TRAVEL

___Great Lakes Airlines offers to the company’s employees and their eligibles flight privileges on all Great Lakes ___Airlines flights, as well as discounted rates on many other airlines. In addition, employees are allowed six ___“Buddy Passes” per year for use by Great Lakes family / friends for discounted fares on Great Lakes Airlines ___flights. Only active employees that have completed an employee eligible form (turned in to the Pass Bureau) ___are eligible for travel benefits. If an employee is on a leave of absence of any kind, all travel benefits are ___revoked until which time the employee is reinstated to active status.

___The personal travel provided to employees and family members is a privilege, not a right of employment or a ___form of compensation. Great Lakes Airlines reserves the right to suspend, revoke or modify these privileges ___in any way, with or without cause or notice, at any time.

___Abuse of travel privileges or misconduct by an employee or family member, including individuals using Buddy ___Passes, while traveling can result in your suspension of privileges, termination of your employment, and/or ___legal action. Travel privileges may also be suspended for a violation of any of Great Lakes Airlines’ policies or ___procedures.

___It is a violation of travel privileges to fraudulently claim a person as a dependent or a family member for the ___purpose of meeting travel eligibility requirements. Other violations include, but are not limited to, the ___following: having a friend check you in for a flight before you arrive at the airport or asking another airline for a ___free pass.

___When planning a trip, expect the unexpected. Please allow extra time to get back home and report for work. ___Failure to report to work on time due to travel problems is not an acceptable excuse and will be counted as a ___tardy or unscheduled absence.

___Employees discharged for misconduct permanently lose entitlement to travel privileges for themselves and ___their family members. This loss of travel privileges includes any travel that might otherwise be available ___because of the discharged employee’s current or future relationship to another employee or retiree at Great ___Lakes Airlines or another airline. It is the responsibility of each Great Lakes Airlines employee to ensure that ___family members who are otherwise eligible for pass privileges are not ineligible because of the circumstances ___described above.

___When can I utilize my flight benefits on Great Lakes?

___After receiving a Great Lakes Airlines employee badge, employees are eligible for unlimited, complimentary, ___space-available travel on any Great Lakes Airlines flight. The employee’s immediate family members also ___qualify for unlimited, complimentary, space-available travel. These family members are referred to as the ___employee’s “eligibles”.

___Who is considered an “eligible”?

___Eligibles consist of:
___• Employee’s spouse
___• Employee’s Domestic partner (see Domestic Partner Policy for requirements)
___• Employee’s children and step-children who are 1) unmarried and at least 50% dependent upon the ___employee and 2) not more than 25 years of age
___• Employee’s parents – either mother and father, mother and step-father, or father and step-mother. You may ___designate only one set of parents per calendar year. Every January, you are allowed to change the set of ___parents you have listed for benefits.
___• In-laws and grandparents are NOT eligible.

___How do I make a reservation to fly on Great Lakes?

___When an employee or eligible travels on a Great Lakes Airlines flight, the employee should “list” for travel ___with any Great Lakes Airlines station agent, or by calling the Pass Bureau in Cheyenne at 800-554-5112.

___What kind of pass will I need to travel on Great Lakes?

___Upon check-in for the flight, the employee or eligible will be given a white “Space-Available Pass” to complete ___and sign. (The employee’s date of hire and employee number will be required when traveling.)

___If I need to travel on Company Business, what do I do?

___If you are traveling on Company Business on a Great Lakes flight, simply call the Pass Bureau to get listed. ___Be sure to mention you are traveling on business and you must state the purpose of your business travel. If ___you need to utilize another airline for your business travel, you must fill out a COBUS Pass Request Form ___and send in to the Pass Bureau.

___What is a Boarding Priority?

___Great Lakes has a list of Boarding Priorities for all passengers that travel on Great Lakes’ flights, whether ___they are revenue or non-revenue passengers.

___When will I get my Buddy Passes?

___After 90 days of employment, the employee will receive 6 Buddy Passes which can be given to friends and ___family members in order to travel on any Great Lakes flight at a reduced rate.

___How do the Buddy Passes work?

___Each Buddy Pass is valid for one ticket. A service charge of $15 will be collected for each flight required for ___travel, depending on the desired itinerary. Only 1 Buddy Pass is required for either one-way or round-trip

___For example:
___• Pax number 1 wants to travel from CYS-TEX via DEN round-trip. Only 1 Buddy Pass is required but the ___cost would be $15.00 from CYS-DEN and $15 from DEN-TEX. So the total would be $30.00 each way or ___$60.00 round trip.

___• Pax number 2 would like to travel one-way from DEN-GRI. One (1) Buddy Pass is still required, but the ___total cost would be only $15.00.

___Listing for the flights is required by calling the Pass Bureau at 800-554-5112. The Buddy Pass may be ___redeemed at any Great Lakes ticket counter. Once issued, tickets are valid for 90 days. Payment is by cash ___or check only and passenger must check in at least 1 hour prior to departure. Travel is on a STANDBY basis ___only. All passengers are boarded in accordance with Great Lakes Boarding Priorities. All revenue ___passengers are of a higher priority and the Buddy Pass traveler may be denied boarding. If the employee is ___traveling with the Buddy Pass traveler, the boarding priority is BP8A using employee’s date of hire. If pax is ___traveling without the employee, the boarding priority is BP8B using current day seniority.

___How can I receive additional Buddy Passes?

___There are 3 ways to receive additional Buddy Passes.

___1) Years of service: Any person that has been employed with Great Lakes a minimum of 3 consecutive ___years, may request an additional 6 Buddy Passes per year. Click here to print off a request for additional ___Buddy Passes.

___2) Exemplary Service: If a customer writes a letter to customer relations regarding excellent service received ___by an employee, that employee will receive one Buddy Pass for each letter.

___3) S.T.A.R. Flight Station: Start the Airline Right. This is for any station that has an originating departure that ___is on time 100% of each month. Each employee of that station receives one Buddy Pass along with a pizza ___party for that month. Each pilots / flight attendant that operated that departure any day of that month will ___receive a Buddy Pass as well.

___What is an ID90, ID95, ID75?

___An ID90 means that you pay 10 percent of the lowest unrestricted fare, ID95 means that you pay 5 percent ___of the lowest unrestricted fare and ID75 means that you pay 25 percent of the lowest unrestricted fare.

___For example: (This is an example only. Please call Pass Bureau for rates)
___• The lowest unrestricted fare from SFO-HNL is a one way fare of $590.00. So your ID90 price would be ___$59.00 each way, plus taxes, ID95 price would be $29.50 each way, plus taxes and the ID75 price would be ___$147.50 each way, plus taxes.

___How do I make a reservation to travel on other airlines?

___To list on any other airline, you will need to call the reservation number for the respective airline.

___How do I get reduced rate passes on other airlines?

___Travel on all other airlines must be requested through the CYS Pass Bureau at least 2 weeks prior to travel. ___Anything requested less than 2 weeks prior to departure cannot be guaranteed.

___How do I request travel on other airlines?

___• Review the Interline Reduced Rate Agreement sheet to determine eligibility, rate and any special ___requirements.
___• Call the Pass Bureau to verify price of tickets
___• Complete an Off Line Pass Request form and send in to CYS Pass Bureau.
___• Include a check or money order, made out to Great Lakes Airlines (unless otherwise stated).
___• Include a self-addressed stamped envelope if requesting tickets returned via mail rather than comat.

___What is the Interline Reduced Rate Agreement sheet?

___In your new hire packet, there was a handout called “Great Lakes Aviation Interline Reduced Rate ___Agreements”, which has a full listing of the airlines we have pass agreements with. ___Where can I get an Off Line Pass Request form?

___You can pick up this request at any ZK ticket counter or click here for a printable version of the form.

___What if I don’t use the ticket I purchased?

___If you don’t use the ticket that you purchased through the Pass Bureau, there are 2 options for you. You may ___send it back to the Pass Bureau for an extension (typically and additional 90 days) or you can get a refund. ___Whichever you decide to do, you MUST send it in PRIOR to the expiration date on the ticket. Once the ___ticket expires, it become null and void – no exceptions.

___Any ticket that you purchase through the Pass Bureau is completely refundable unless otherwise stated. To ___request a refund, send in the unused ticket, along with a self-addressed stamped envelope, to the Refund ___department in CYS. They will process your refund and send a check back to you within 4-6 weeks.

___Is there a dress code policy to travel on Great Lakes?

___Yes there is a dress code to travel as a non-revenue passenger on Great Lakes flights along with any other ___airline. To find out other airlines’ dress code policies, you will need to contact that airline directly.
